Legg-Calve’-Perthes Disease (LCPD)


The cause of LCPD is unknown.  A strong genetic influence has been identified in Toy Poodles and Manchester Terriers.  LCPD begins following an interruption of part on all of the blood flow to the femoral head from the ring of epiphyseal vessels.  Avascularity causes death of osteocytes and marrow cells but the “dead bone” remains mechanically sound and functional until remodeling begins.  Remodeling weakens the bone predisposing it to compression fracture and alterations of the contour of the femoral head.  In clinical cases OA, pain and muscle atrophy rapidly ensue.

A.Occurs in miniature and toy-breed dogs.

B.Clinical disease is typically noted from 4-11 months with a peak occurrence between 6-10 months.

C.Males and females are equally affected.

D.LCPD is bilateral in 12-16% of cases.
   
E.Clinical Signs

1.Intermittent to acute non-weight bearing lameness.

2.Pain on hip extension.

3.Mild to moderate muscle atrophy.   

4.Many cases have concurrent medial patellar luxation.

F.Diagnosis

1.Clinical presentation.




2.Standard radiographic views of the hips.

a.Increased radiolucency in areas of the femoral head (“mottled” femoral head)

b.Flattening of the femoral head with thickening of the femoral neck.

c.Hip OA

d.Marked muscle atrophy

G.Treatment

1.Conservative management using a sling and activity restriction of early cases is usually not successful (less than 25% improve).

2.Femoral head and neck excision is the treatment of choice.

a.Must remove all fragments of the femoral head and neck.

b.Aggressive postoperative physical therapy and anti-inflammatory meds may be necessary to achieve a good result.
H.Prognosis

1.Reported to be good to excellent.

2.Be cautious giving an excellent prognosis in a tiny dog with severe muscle atrophy.

3.Warn owners preoperatively that meds and physical therapy may be required to help achieve a successful outcome.
